Our system retains your Facebook Identity* to ensure that only you will have access to your information. Follow the steps below, and your Facebook Identity will be removed from our system

Here are the steps:

1. Login into the Patient Portal using your Facebook Login
2. Click on “My Profile” on the left side
3. Click on “Change Login”
4. Click on “Click here to change details”
5. Send yourself the One Time Passcode for security
6. Type the code into the box, and press Continue
7. You can now choose to Register with Google, Apple or Username and Password
8. Your Facebook identity is now removed from our system

* Note, there is no other data retained from your Facebook Profile other than your Facebook Identity
